New Student Registration
New students must come to school with a parent or guardian to complete registration.  Parents and student can complete the forms below ahead of time and bring them to the school office along with a copy of the student's birth certificate, social security card and custody papers (if applicable).

 


Open Enrollment
Hardin-Houston does accept Open Enrollment students.  If you wish to Open Enroll into our district, complete the form below and submit it to the Superintendent's Office at 5300 Houston Road, Houston, Ohio 45333 for review

 


National Lunch Program (Free & Reduced)
To apply for free or reduced breakfast/lunch prices, complete the form below and submit it to the Superintendent's Office.
 

 


Facilities Rental Agreement
Hardin-Houston is pleased to make our facilities and grounds available to our community for their personal event needs.  If you would like to use one of our buildings or ball fields, please complete the Rental Agreement Form below, print it and bring it to the principal's office of the building/grounds you wish to use.
